Insurance Information -
Commercial Insurance:
Dr.
Barrows participates in several
health plans, however, it is
recommended that you contact the
number provided on your insurance
card for the most up to date and
accurate information. In addition,
please be sure to obtain insurance
advance pre-authorization for
specialty physician evaluation and
treatment if your managed care
health policy requires this,
otherwise your appointment may need
to be rescheduled.
Medicare and Medicaid:
Medicare assignment and straight
Title 19 are accepted.
Personal Injury and Motor Vehicle
Accident Claims:
Patients are responsible for keeping
their accounts current. As some
injury cases can take months to
years to settle, it is strongly
recommended that you also stay
within the guidelines of your health
or automobile insurance policy. Any
medical legal fees, such as medical
legal testimony or reports requested
by an attorney, must be paid for in
advance.
Wisconsin Allied Physician Association (WAPA):
Insurance contracts with the
Wisconsin Allied Physician
Association have not been renewed
since the dissolution of WAPA on
December 31, 2004..Please contact
your insurance provider for new
contracted health care providers.
Workers
Compensation:
Please be sure that an initial
injury report has been filed with
your employer. In addition, worker’s
compensation insurance information
will be required in order to file
your claim. Due to the risk of
denial of coverage under Workers
Compensation, it is strongly
recommended that you follow
requirements of your general health
insurance policy, including
obtaining physician referral if
required.
